Enrollment Process
Please call and schedule a tour and interview. Once you have determined that The Giving Tree is the right program for your family and we have an available position of care we will have you fill out the paperwork below. Each form should be printed, reviewed and then signed. All pages must be complete for a position to be secured.
We offer positions of care on a first come first served basis. Infant positions can be secured in advance and you should inquire about our availability.
We will need a copy of your child's birth certificate and insurance card for your child's file.
For our Preschool students, we also need a copy of their most recent vision and hearing screening.
Below is a link to our parent handbook. We will need your initial on pages 8, 10, 12 and 24 to complete your child's file.
